Media Release Consent

I hereby grant permission to Mothers’ Milk Bank of the Western Great Lakes (“Milk Bank WGL”) to use my/my child(ren)’s likeness in a photograph, video, or other media, in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all Materials will become the property of Milk Bank WGL and will not be returned.

I hereby authorize Milk Bank WGL to edit, alter, copy, exhibit, publish, or distribute these Materials for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein the Materials are used.

I hereby hold harmless, release, and forever discharge Milk Bank WGL, Milk Bank WGL’s representatives, employees, officers, and directors from all claims and causes of action including, without limitation, all claims for invasion of privacy, infringement of my right of publicity, defamation and any other personal and/or property rights which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. I understand that I may revoke this authorization at any time by notifying Milk Bank WGL in writing. The revocation will not affect any actions taken before the receipt of this written notification.
